Fortress Software Inc. (FSI) has created this privacy statement in order to demonstrate our firm commitment to your privacy. The following discloses the information gathering and dissemination practices used with-in our organization, our Friendomatics Fundraiser, and on our web site.
Matchomatics uses an order form for customers (schools) to request the necessary answer forms (questionnaires) or an onine survey for participation in the Matchomatics fundraiser. This order form requests your name; your organization's name; your school's name, address, city, province/state, and postal/zip code; your school's telephone number; your mobile telephone number; your e-mail address; your advisor's name, mobile telephone number, and their e-mail address.
Contact information from the Matchomatics order form is used to ship the necessary forms to the requester or to set up an online survey for the requester and to get in touch with the requester when necessary.
We may also use this contact information to update our database of individuals or organizations interested in the Matchomatics fundraiser.
Requesters and schools may opt-out of receiving future mailings from us at any time; see the choice/opt-out section below.
Participation in the Matchomatics Compatibility Fundraiser requires participants to fill out a Matchomatics Answer Form or Online Survey. This form or online survey requests a name, and optionally a gender, birth date, grade, and answers to a Matchomatics Questionnaire which can be customized at any time by the school requesting it.
All information provided to FSI on the Matchomatics answer forms and online surveys are kept strictly confidential. This information is used to create the Matchomatics Result Forms and nothing else. FSI does not share or sell any information obtained from the answer forms or online surveys to any company or government entity. Once scanned into our database, paper answer forms are destroyed and all information from the answer forms and online surveys are deleted from our database at the end of every school year.
FSI provides schools the opportunity to opt-out of receiving promotional/marketing information from us.
